Due to continuing changes in the healthcare industry, we would like to provide you with our practice Billing Policies and how they relate to you.

If we do not  participate with your insurance carrier: We will file the claim if you  provide us with the following information: Name and mailing address of your carrier, policy number, group number policy holder's' full name, policy holders' date of birth and social security number. We file these claims as a courtesy to our patients so that your carrier reimburses you in a timely manner. Because your policy is a contract between you and your carrier we will look to you for payment not your carrier.

Co-Pays: If you have an insurance that  requires a co-pay, it must be paid the day of your appointment.

Lab Work: Please be advised that many insurance companies require you to go to a certain lab and/or hospital. You will be responsible to pay your bill if you go to a lab and/or hospital that is not covered by your carrier. Please check with your insurance company to see where you should go for these services.

Insurance's that require referrals:
If you have an insurance company that requires a referral from your primary care physician it is your responsibility to obtain the referral and keep track of its expiration. We will be able to see you for scheduled appointments if we do not have the referral on/or before the date of the appointment.

Disability Forms: Upon receipt of a disability form(s), our office will complete and mail the form(s) within 5 business days. There is a $10.00  charge for each disability form, except for Workmen's Compensation for which there is no charge.

Hand Therapy and Work Therapy: Hand & Upper Extremity Surgery, P.C. is a private practice which contracts independently with insurance carriers. Our Hand Therapy and Work Therapy departments are part of Kaleida Health and abide by their billing policies and procedures. A separate co-pay is required for Hand Therapy and Work Therapy.
