Prior to Arrival...
You will be given a packet of information that includes the information contained in this section of the website as well as a "Pre-Admission form.  Please complete the form and return it to us.  It is important to fill in all the information.
 

PLEASE BRING THE FOLLOWING ITEMS WITH YOU FOR ADMISSION:

  • Insurance card for the patient
  • Insurance referral (if applicable)
  • Co-payment (if applicable)
  • Legal proof of custody (court documentation) if you are not the parent.
  • If you have "PRE-CARE", be sure to call your insurance company before your admission.
  • If your child is scheduled for surgery and you are not able to mail the information to us, please call the Admissions Department at 878-7206 or 878-7208 and we can take your information over the phone.

If you have any financial concerns regarding your child's hospitalization, our Patient Accounts Department will be pleased to assist you. They may be reached by calling 878-7781 Monday through Friday from 8 AM to 4 PM.

On the day of your child's surgery, you should report directly to the admissions department located in the Same Day Surgery Unit on T9.
